Clarify four issues before choosing a visa
For healthcare applicants, the profession, recognition, language and professional authorization are more important than starting with a visa label.
Medicine, dentistry and professional nursing are regulated and require the relevant recognition/authorization.

Nursing normally B2; doctors and dentists generally B2 plus a specialist-language exam aligned with C1.

Residence routes that may be relevant
Skilled worker — Sections 18a / 18b
For applicants with the required recognition and a qualified job offer. Fully recognized nurses commonly examine Section 18a.
Recognition — Section 16d
For cases requiring compensation measures or where a recognition partnership with an employer is available.
Vocational training
The general visa benchmark is B1; a separate secondary job can be possible up to 20 hours per week.
Opportunity Card / job search
Potentially useful without a job offer, but in regulated healthcare work it does not replace recognition and licensing.

Family, insurance and long-term residence
Spouse & family
Current official guidance generally does not require proof of German for a spouse joining a skilled worker with a valid residence title; the spouse can generally work after receiving the relevant family title.
Insurance & social security
Once employment subject to social insurance begins, health, pension and other social contributions form part of the employment system. Transition coverage should match the visa requirements.
Settlement permit
The general skilled-worker route can be available after three years and 36 months of pension contributions, together with the other conditions. Facilitated rules for German graduates and Blue Card holders are separate.
Frequently asked questions
Does partial recognition end the process?
Not necessarily. An adaptation programme, knowledge examination or another compensation measure can be available depending on the profession and authority decision.
Can I enter Germany before recognition is complete?
In some cases yes. Section 16d routes and the recognition partnership are designed for defined situations where recognition continues after entry.
Can a professional nurse obtain an EU Blue Card?
For current planning, the official nursing guidance states that the Blue Card does not apply to nursing professionals; use the nursing-specific skilled-worker and recognition routes unless official rules change.
Does the spouse of a skilled worker need A1?
Current official guidance says proof of German is not required for family members joining a skilled worker with a valid residence title; the principal holder’s residence category still needs to be checked.
